Case Management is a service offered to families that help guide them on the path of finding the right resources in the community when dealing with the Department of Health and Human Services and other service providers.
Determine the clients needs and match the request to the proper channels
Carry on request for action and follow up with the proper departments
Visit the clients home and surrounding environments to determine the functionality of the services requested by the client, resources available
Note any risks posed to the clients and service providers
Determine the level of support available in the community and accommodations to recommend.
Create a schedule of services with the provider's contact information
Follow up with the provider and client before and after services to determine if services met a quality level of satisfaction and resolve issues
Help the client answer questions, resolve issues, and maintain a quality level of service
